Elective Agreement Form
An
Elective Agreement must be filled out before starting
EVERY elective class for which elective credits are
to be used.

Reimbursement Receipt Form
When
requesting parental reimbursement for classes taken
through an approved organization by the Greater San
Diego Academy, a Reimbursement Receipt form is
required. An Elective Agreement must be completed
for each payment and an original receipt must be
attached.

It's Easy to offer an Elective Class...
It's easy to offer an elective class for students
of Greater San Diego Academy...
- Develop a learning plan, "What do you want the children to learn?"
- How many classes would it take to complete the study? Is it just a one-time class or will it last nine weeks?
- Figure out your total cost...What materials do you need to purchase?
- How many students would be your minimum? Maximum?
- Take the total cost for your time and materials, divide it by the number of students and/or classes, and calculate the student price for your elective class(es).
- Providers of elective classes take full responsibility for the safety and well-being of all participants. They must invoice the school for payment of student unit credits.

Now are you ready to submit a proposal to Greater
San Diego Academy?
- When a parent requests your services, that parent will provide you with an Elective Voucher to be signed. We will then send you a W-9 form for tax purposes and request a background check if that is the parent's desire. (We suggest parents ask for a background check for all elective providers unless they will remain on site with their children.)
